Telehealth Informed Consent

Important information about receiving care remotely through independent licensed providers accessed through the Sinta platform.

Telehealth is not emergency care

If you believe you are experiencing an emergency, call 911 or go to the nearest emergency department. Do not wait for a response through the Sinta platform.

This document describes telehealth, its expected benefits and material risks, and your rights. Your provider may ask you to confirm this consent electronically before a visit. It does not replace the discussion you may have with your provider about your care.

1. What telehealth is

Telehealth uses electronic communications to allow a patient and licensed healthcare provider in different locations to exchange health information and deliver clinical services. It may involve live video or audio, secure messaging, electronic questionnaires, photographs, medical records, laboratory information, prescriptions, and other digital tools.

2. Sinta and provider roles

Sinta supplies technology and administrative coordination. Sinta does not practice medicine or direct clinical judgment. Medical services are provided by independent licensed professionals, including providers made available through OpenLoop and/or its affiliated professional entities. Your provider is solely responsible for evaluation, diagnosis, treatment recommendations, prescribing, follow-up, and determining whether telehealth is appropriate.

A provider-patient relationship is established only when an independent provider accepts you for care under applicable law; using the Sinta website or creating an account alone does not create that relationship.

3. Potential benefits and alternatives

Telehealth can improve convenience and access, reduce travel, support faster communication, and help coordinate clinical, laboratory, and pharmacy workflows. Potential alternatives include an in-person visit, seeking care from another provider, using a local urgent-care or emergency facility, or declining the service. Your provider may recommend or require in-person evaluation.

4. Risks and limitations

Telehealth has limitations and risks, including:

  • Technology failure, poor connection, interrupted communication, or delayed access;
  • Reduced ability to perform a physical examination or observe certain clinical signs;
  • Incomplete, inaccurate, or misunderstood information affecting clinical decisions;
  • Delays in testing, diagnosis, prescriptions, shipping, or follow-up;
  • Privacy or security risks despite reasonable safeguards; and
  • The possibility that telehealth will be determined inappropriate and in-person care will be required.

No specific clinical result is promised. Treatment, medication, laboratory, and wellness outcomes vary by individual.

5. Privacy, confidentiality, and recordings

Sinta and participating providers use reasonable privacy and security safeguards. Health information is handled under applicable law, provider privacy notices, Sinta's Privacy Policy, and Sinta's role as a HIPAA Business Associate when applicable. Electronic systems cannot be guaranteed completely secure.

A telehealth session may be recorded for quality, safety, training, or documentation purposes only when the recording is disclosed and the consent required by applicable law has been obtained. If a proposed recording is optional, you may decline without losing access to otherwise available care. You agree not to photograph, screen-capture, or record a provider or session without the prior consent of everyone involved.

6. Your responsibilities

You agree to provide complete and accurate identity, location, health, medication, allergy, and symptom information; participate from a reasonably private and safe location; maintain a reliable device and connection; and tell the provider if you do not understand a recommendation. You should not participate while driving or operating machinery.

You must tell the provider where you are physically located at the time of a visit. Providers may deliver care only where they are appropriately licensed or otherwise authorized. If your location changes, a scheduled provider may be unable to treat you.

7. Clinical decisions, prescriptions, and testing

Your provider decides whether to diagnose a condition, order testing, recommend treatment, or prescribe medication. A request does not guarantee approval or access to any medication or program. Providers may require additional records, laboratory work, identity verification, follow-up, or in-person evaluation.

Use medications only as directed, review pharmacy materials, and report adverse reactions or changes in your health promptly. Contact your provider for clinical questions and seek urgent or emergency care when appropriate.

8. Cash-pay services

Programs accessed through Sinta are entirely cash-pay. Sinta does not submit claims to insurance and does not promise reimbursement. Charges may include clinical services, medication, laboratory testing, fulfillment, shipping, technology, and other disclosed items. You are responsible for reviewing and accepting charges before purchase.

9. Consent and withdrawal

By electronically accepting this consent or proceeding with a telehealth visit after receiving it, you acknowledge that you understand the nature of telehealth, its benefits, material risks, alternatives, and the independent role of your provider. You have had the opportunity to ask questions.

You may withdraw consent to future telehealth services at any time by notifying your provider. Withdrawal will not affect care already provided, records already created, or uses of information already made in reliance on your consent. Withdrawing may limit access to services that can only be delivered through telehealth.
